I hereafter referred to as the adopter, hereby agree that the above described animal is being adopted by me solely as a pet for myself and/or my immediate family and agree to bring new rescue to your Vet within 60 days after adoption. I agree that I will not sell, give away or otherwise dispose of said animal to any person(s), dealer, retailer, auction, institute or any other entity for any reason. We will immediately notify CK if this animal is lost or stolen or if we are unable to keep this animal at any time. We won’t relinquish this animal to another party without the written consent of CK. If at a later date I am unable or unwilling to keep this pet, I agree to first contact Compassion Kind (CK) and give them the option to reclaim said pet at no charge.

I hereby agree to care for the above-described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water and veterinary care. We will provide proper nourishment and medical care including monthly heartworm, flea prevention and regular grooming. I further agree that said pet shall reside inside my home and shall not be allowed to roam freely. I hereby agree that this pet shall wear a collar or harness with identification tags at all times.

I hereby understand and agree that animals adopted under the CK Adoption Program are intended to be cared for and placed with the adopter for the remainder of the animal’s natural life. If adopter decides to end the adoption for any reason other than death, the animal shall be returned to CK within 14 days’ notice from adopter. Animal must return with updated vaccines and medical records. No fees from adoption or otherwise will be refunded to Adopter/s.

I hereby agree that all fees and expenses, including adoption fees and expenses and those incurred as a result of repossession, including any legal fees, are non-refundable and the organization is not liable for reimbursement, subject only to exceptions noted in this Agreement. I hereby agree that continued possession of said animal is contingent upon Adopter fulfilling all obligations, terms and conditions pursuant to the signed contract for said animal and any breach of the adoption contract requires the immediate return of said animal to CK.

I hereby agree that the adoption fee helps to cover the rescue of additional animals and expense of rescuing and caring for the animal and is not refundable. I hereby understand and agree that CK makes no representations or warranties, expressed or implied, about the above mentioned animal's temperament and is hereby absolved from any liability for future damages or injuries caused by said animal.

I certify that all statements made by me on this adoption agreement are true and correct. I agree that CK has the right to confiscate the above-described animal in the event that any statements made by me are found to be false, that said animal is not being cared for in a way that best suites the animals welfare and/or my check for the adoption fee is returned for insufficient funds.

I recognize that although CK has made an effort to provide an animal which is compatible with my home environment, the animals are in CK foster care for a very short period of time. Therefore CK cannot and does not warrant the temperament or behavior of this animal. We release CK and its members from all liabilities that may result from our acquisition of this animal.

CK is authorized to remove this animal from my home if it is judged not to be receiving proper care or if there is a violation of this agreement. We agree to permit CKR to visit the home where the animal is kept given reasonable notice and that such entry shall not constitute a trespass.
